This isn't a task-management role. It's a clinical leadership position at the centre of everything.

At Qualicare Burlington, the Nurse Manager is the clinical backbone of our care model. You're the person who conducts every Aging Well Advisory assessment, builds every care plan, and makes the calls that protect our clients when their situation gets complex. You're also the clinical anchor for our care team — the standard they measure themselves against.

We've spent 12 years building something different in Burlington and Oakville: a home care practice that engages families early, before a crisis forces their hand. That philosophy only works when the clinical leadership behind it is sharp, trusted, and genuinely invested in what happens to the people we serve.

This role is for a nursing leader who wants real ownership — not just a title.

What You'll Own

•      Clinical assessment and care planning — You assess every new client personally. Your care plan is the foundation everything else is built on.
•      Complex case management — Dementia progression, post-hospital transitions, palliative referrals. You lead these conversations with families and make the calls that matter.
•      Care plan currency — Every active client's plan is reviewed quarterly. You own the standard and the execution.
•      Clinical oversight of our care team — PSW competency, escalation support, incident documentation. The Care Quality Lead works alongside you; the clinical judgement is yours.
•      Leadership of our Visiting Nurse and Dialysis Nurse teams — You are the clinical point of accountability for these specialized services, ensuring coordination, standards, and continuity across every visit.
•      Cross-functional collaboration — You work closely with our Director of Community Care Integration on referral partner relationships, and alongside the Owner on strategy as we grow.
•      Culture carrier — The values of this organization live or die by how clinical leadership shows up. You set the tone.

What We're Looking For

•      RPN or RN in good standing with the CNO
•      Community or home care experience — you understand the complexity of care outside hospital walls
•      Clinical confidence — you make decisions in ambiguous situations and document them clearly
•      Communication that families trust — you can hold a hard conversation with a family at 6pm and still be the person they're grateful for
•      A belief that early engagement prevents harm — not just something you say; something you've seen play out
•      Asset: Gentle Persuasion Approach (GPA) Trainer Certificate — we're building a dementia care culture, and someone who can train to that standard accelerates everything

Why This Role, Why Now

We're at a deliberate growth point. We're expanding our Aging Well Advisory model, deepening relationships with Halton-area referral partners, and building the clinical infrastructure to support that growth. The Nurse Manager we hire now will help shape what that looks like — and grow with it.

This is a full-time salaried position working directly with the Owner. As we scale, this role scales with us — including a clear pathway to a senior clinical leadership position when we reach the next threshold.

Compensation is competitive and discussed transparently at first conversation. We don't make you jump through four rounds to find out if the numbers work.
