What You’ll Do

  • Lead and mentor our front line heroes: our Care Providers.

  • Elevate customer experience with confidence, empathy, and professionalism.

  • Partner with hospitals, physicians, and families for seamless client care.

  • Support new and emerging programs that strengthen community-based care.

  • Help create a culture where everyone feels supported and inspired.

What You Bring

  • Registered Nurse (RN)/Registered Practical Nurse (RPN) in good standing with CNO.

  • Leadership experience in community and home health.

  • Strong communication, critical thinking, and collaboration skills.

  • A passion for helping clients live well at home.
