About Qualicare and the Role
At Qualicare Home Health, we pride ourselves on providing patient-centered care that promotes recovery and independence at home. We are seeking a motivated Physical Therapist Assistant (PTA) to join our growing team in San Ramon. Working under the supervision of a licensed PT, you will deliver individualized therapy treatments and assist clients in achieving their mobility and functional goals.
Key Responsibilities
- Implement physical therapy programs established by the supervising PT
- Assist patients with therapeutic exercises, mobility training, and safety education
- Observe and report patient responses and progress to the PT
- Reinforce patient and caregiver education regarding home exercises and adaptive equipment use
- Document all treatments accurately and promptly in EMR (WellSky)
- Communicate effectively with the care team to ensure quality outcomes
- Follow established infection control and safety procedures during in-home visits
Qualifications
- Active, unrestricted California PTA license
- Associate degree from a CAPTE-accredited Physical Therapist Assistant program
- Minimum 1 year of clinical experience (home health or outpatient preferred)
- Current CPR certification
- Reliable transportation and valid California driver’s license
- Excellent communication, time management, and documentation skills
- EMR proficiency preferred (WellSky or similar)
