About Qualicare and the Role
At Qualicare Home Health in San Ramon, we provide compassionate, high-quality Home Care and Home Health services. We are seeking a dedicated Occupational Therapist (OT) to join our team. In this role, you will assess patient needs, develop and implement individualized therapy plans, and deliver interventions that help clients improve daily living skills, regain independence, and enhance their overall quality of life.
Key Responsibilities
- Perform initial evaluations to assess ADL/IADL capabilities and home safety risks
- Develop goal-oriented treatment plans to enhance independence in self-care and daily tasks
- Address physical, sensory, emotional, and cognitive barriers to function
- Educate patients and caregivers on adaptive techniques and assistive device use
- Recommend home modifications to reduce risks and improve accessibility
- Document care accurately in the EMR (WellSky) and maintain compliance with regulations
- Collaborate with PTs, RNs, and interdisciplinary team members for coordinated care
- Participate in team meetings, QA initiatives, and case conferences
Qualifications
- Active, unrestricted California Occupational Therapist (OT) license
- Minimum 3 years of clinical experience (home health or geriatrics preferred)
- Current CPR certification
- Reliable transportation and valid CA driver’s license
- EMR documentation experience preferred
- Strong communication, adaptability, and problem-solving skills
