Job Summary
The CHHA will provide life changing service to our patients and their families. The role is ideal for someone looking for flexible, as needed work now, with the opportunity for more hours once we begin serving patients.
Key Details:
- This is a PRN/as-needed position, with patient care expected to begin in the near future.
- Initial responsibilities will involve helping meet licensing requirements before patient visits start.
- Those hired now will have priority consideration for full-time or part-time roles once we are fully operational.
Responsibilities
- Provide personal care (bathing, grooming, dressing, toileting, transfers) under an RN‑ or LVN‑supervised Plan of Care.
- Offer companionship, light meal prep, and safe‑environment housekeeping.
- Take and record vital signs; report condition changes via WellSky mobile app.
- Reinforce patient/caregiver education on ADLs, skin integrity, and fall prevention.
- Document all care in real time—no end‑of‑night charting marathons.
- Partner with nurses, PT/OT/ST, and social workers in weekly case conferences.
Requirements
- Active California CHHA certificate & BLS/CPR.
- Reliable vehicle, valid CA driver’s license, and up‑to‑date auto insurance.
- Comfort using a smartphone or tablet for documentation.
- Physical ability to safely assist with transfers/lifts up to 35 lb.
- Able to lift/transfer 150 lbs or more as needed.
- Able to bend, stoop, climb stairs, and stand/walk for extended periods.
- Communication: Must be able to speak, read, and write in English effectively.

Preferred (not required): six + months of home‑health or hospice aide experience; bilingual (Spanish, Tagalog, Farsi, etc.).
