🏥 What You’ll Do
As the Clinical Manager, you will ensure quality patient outcomes while supervising and mentoring a multidisciplinary care team. Responsibilities include:
- Lead, support, and coach RNs, LVNs, HHAs, and therapy staff
- Oversee and evaluate the execution of personalized care plans
- Develop, implement, and assess clinical care protocols and outcomes
- Ensure compliance with Medicare, Medi-Cal, Title 22, and accreditation standards
- Support clinicians with accurate OASIS documentation and ICD-10 coding
- Maintain WellSky, EMR documentation proficiency
- Supervise new clinical employee orientation and competency
- Participate in the on-call rotation and provide field care as needed
- Prepare for and participate in CDPH, CMS, and accreditation surveys
- Collaborate with Intake, QA, and Scheduling to ensure operational excellence
- Analyze clinical performance data and propose process improvements
✅ What We’re Looking For
We’re seeking a compassionate, results-oriented RN with a strong clinical and leadership foundation.
Minimum Requirements:
- ✅ RN Licensure: Active and in good standing with the California BRN (Required)
- 🕒 Experience: 3+ years clinical experience, including 2+ years in a supervisory or management role (Home health preferred)
- 📋 Knowledge: Strong grasp of California Title 22, Medicare Conditions of Participation, and home health regulations
- 💬 Skills: Excellent communication, coaching, organizational and analytical skills
- 💻 Technology: Proficient in EMR (preferably WellSky/Kinnser)
- 🧭 Leadership: Ability to lead through change, engage teams, and foster accountability
📅 Schedule & Availability
- Full-time: Monday–Friday
- Flexibility required for after-hours calls (as part of a shared on-call rotation)
- Hybrid administrative/field role with occasional in-home patient visits
